Outcomes Of Chronic Hepc G3 Cirrhotics Treated With Pegintron And Ribavirin

 INTRODUCTION: Treating HCV cirrhotics is very controversial and difficult.There are side effects of treatment and poor tolerability with questionable efficacy.It is very pertinent to identify non responder patients early to avoid unnecessary prolonged treatment.Rapid virological response (RVR) is defined as HCVPCR negative after 4 weeks of Peginterferon and Ribavirin therapy and is highly predictive of sustained virological (SVR).we wanted to study the prognostic value of RVR in HEPC G3 compensated cirrhotics. 

AIMS & METHODS: This study was conducted in our institution from January 2007 to January 2008.50 consecutive Hep C G3 compensated cirrhotics (child B)were included in this study.They were given 1 ucg/kg of pegintron/week  and 400 mg BID of Ribavirin.Treatment was given for 24 weeks.These patients were seen @ 4 weekly basis.They had HCVPCR done @ week , 4 , 12, 24 and 6 months after completing the treatment. All patients were G3 , mean age 48. 35 were males and 15 were females. 

RESULTS: Out of 50 patients 5 patients lost to F/U.22 out of 50 (44%)patients were HCVPCR negative @ week 4 and 24.This was labelled as group A.After 6 months completion of therapy 20 patients (40%) were HCVPCR negative (SVR) .

Twelve  out of 50 (25%) patients were negative @week 12 and week 24.This group was labelled as group B.Six  months after completion of therapy 8 patients (16%) were HCVPCR negative (SVR).Nine  patients were HCVPCR positive after 12 weeks of therapy and were labelled as non responders. 

CONCLUSION: Overall treatment respose is significantly lower in HEP C G3 Cirrhotics.4 week stopping role in this patient population may be considered to avoid unnecessray complications of treatment.Further studies are needed to confirm these observations.
