Rapid Virological Response Is A Predictor Of Sustained Virological Response In Chronic Hep C G3 Patients Treated With Pegasys And Ribavirin

INTRODUCTION: Genotype (GT) is generally considered the most important predictor of sustained virological response (SVR).Role of viral clearance @ week 4 i.e Rapid virological response (RVR)   to  predict  SVR is not adequately studied in Chronic HepC G3 patients.We wanted to study the importance of Rapid virological response (RVR) to predict SVR in chronic Hepc G3 patients. 

AIMS & METHODS: A prospective study was conducted in our institution between January 2008 to January 2009.150 consecutive chronic Hep C G3 patients eligible  for  treatment were  enrolled in this study.They were given 180ucg of pegasys  weekly along with Ribavirin 400mg twice daily.Tratment duration was for 6 moths.They were seen in outpatient clinic on 4 weekly basis.Quantitativ HCVPCR was done @ baseline @week 4,week 12 and week24 during treatment and then 6 months after the completion of treatment to establish SVR.RVR was defined as HCVRNA <50IU/ml @week 4  of therapy Copmlete EVR was defiened as HCVPCR <50IU/ml at week 12 of therapy and partial EVR was defined as >2log10 drop in HCVPCR @week 12 of therapy.

RESULTS: Out of 150 patients 140 were available for analysis .Median age was 44 , median ALT was 90 ,median HCVPCR was 850,000 IU/ml.Logistic regression analysis was done .RVR was considered independent/predictor  variable and SVR (sustained virological response) was considered dependent /outcome variable.100 0ut of 150 (67%) patients had RVR and SVR  in that group was 88%.30 patients had EVR( 20% )and SVR was 60%.

CONCLUSION: Rapid virological response (RVR) is a good predictor of Sustained virological response (SVR) in HCV G3 patients .Patients who do not have RVR but EVR may need therapy for 9 months to a year.Further studies are needed to confirm this observation.
